The processing of the crew and passengers of civilian vessel 6-0-SPD occurred largely without incident. Performance of military personnel in general is deemed adequate. However there was a notable lapse that almost lead to a "plague ship" style incident occurring.
 
==== Error #01 ====
 
  *Exhaustive investigation into the “close call” incident has identified that the first point of failure occurred during the initial escort of passengers from ship to hospital.
  *The ship was completely swept for remaining passengers as per quarantine procedure. No outstanding prisoners were found. This corresponded with life-sign data later obtained from ship records.
  *Later counts of //military personnel// turned out one additional person. This information was ignored at the time. This should have immediately raised suspicion and relevant action to detect impersonators.
 
==== Error #02 ====
 
  *Military guards assigned to monitor the interior of the 6-0-SPD did not all perform patrols in groups of a minimum of 2 as is described by standard military protocols.
  *This allowed a window for one “guard” to have unmonitored access to core ship computers for an excessive length of time.
  *Corruption of ship systems was later traced back to this single point of failure.
 
==== Error #03 ====
 
  *Captain Gabrielle Sheer was allowed to remain on the bridge alone for significant portion of time.
  *This allowed the impostor opportunity to speak with the Captain with a goal of persuading her to leave the system with the ship in tow.
  *The Captain assured this investigative team that they disagreed in the strongest of terms, but the fact that this information only came to light due to the Captain’s good conscience is particularly damning.
 
==== Error #04 ====
 
  *Panic following the recapture of the 6-0-SPD resulted in a critical lack of coordination.
  *It is noted that recapture upon unauthorized fast-as-light spooling maneuvers was handled well despite the obvious surprise that this event caused. The direst consequences of a plague ship escaping was prevented by this swift action.
  *However, capture and monitoring of the guilty party did not come up to regulation standard. Emergency cure was administered by doctors but the culprit again managed to evade incarceration and has since blended into civilian personnel.
 
==== Final Reccomendations: ====
 
  *There were repeated severe failures in procedure, any one of which could have caused a local or disseminated catastrophe.
  *It is appreciated that this was a novel situation with which military personnel do not have a vast amount of experience.
  *We recommend the Military platoon involved undergo repeated basic protocol training until the fundamental errors no longer occur.
  *The culprit is considered unknown and at large at this point. Visual identification proves useless, it appears several disguises were used. We can only hope that any future damage caused by this criminal does not occur on our watch in the future.

We interrupt your regular programming to bring you an emergency broadcast. A planet wide viral attack has been launched against our planet of Voraxi. Cybernetics across the whole planet have been infected and may cease to function at any time.
 
We ask the people of Voraxi not to panic and follow these simple instructions.
 
Firstly power off and remove any cybernetics you currently have installed. The virus appears to be linked to cybernetics and spreads through them. Failure to power down cybernetics could lead to loss of control or even minor explosions. The public is urged to spread this information to friends and neighbours as soon as possible in order to reduce the damage. If you need help removing cybernetics then please visit your local Organics community centre as they have generously offered their services for free during this period of planetary crisis.
 
Please only visit nearby hospitals if you have been significantly injured.
 
As of yet we do not have information as to who is behind this attack. Code experts have detected a signature of sorts - “KLTR”. We have not heard of this “KLTR” organisation before and urge them not to pursue this destructive course of action any further.
 
In the meantime this planet has been placed under quarantine until the crisis has been averted. So please do not try to use your orbital shuttles at this time. Your cooperation is appreciated.
